Abstract
Formål: Studiens primærhensikt var å beskrive pasienters erfaringer med sykepleiernes bidrag i en rehabiliteringsprosess. Sekundærhensikten var å undersøke om Kirkevold (1997) sitt teoretiske perspektiv på sykepleieres funksjoner i akuttrehabilitering til slagpasienter er egnet til å fange essensen av pasienters erfaringer med sykepleien i en bredere rehabiliteringskontekst.
Teoretisk forankring: Sentrale teoretiske perspektiv og begreper knyttet til; pasientens behov i rehabilitering, rehabiliteringsideologi, rehabiliteringsprosessen, tverrfaglig samarbeid og rehabiliteringssykepleie dannet det teoretiske grunnlaget. I tillegg ble Kirkevolds (1997) teoretiske perspektiv benyttet.
Metode: Studien hadde et kvalitativt, hermeneutisk design. Metode for datainnsamling var halvstrukturerte intervju. Åtte informanter som hadde erfaring fra en rehabiliteringsprosess ble inkludert. I analysen ble det benyttet systematisk tekstkondensering i fire trinn.
Resultat: Funnene viser at sykepleieren var avgjørende for utfallet av dagen og rehabiliteringsprosessen for den enkelte pasient og ble tolket som fundamentet i rehabiliteringsarbeidet. Samtidig ble ikke de sykepleiefaglige bidragene definert som rehabiliterende. Utsagnene indikerte en forståelse av behandling og terapi som rehabilitering mens ”det andre”, det vil si sykepleieomsorgen ble ansett som atskilt fra rehabiliteringsarbeidet. Informantene savnet mer tid med sykepleierne, at de utfordret mer, og at de i fellesskap satt mål for prosessen.
Konklusjon: Sykepleieren er avgjørende for rehabiliteringsprosessen, men det er behov for ytterlige kompetanse innen feltet. Områder for videre kompetanseutvikling er etablering av en rehabiliterende kontekst med vektleggelse av sosiale fellesskap, kommunikasjon og tverrfaglig samarbeid. I tillegg må sykepleierne ha økt fokus på å stille krav, sette grenser, utarbeide mål i samarbeid med pasient og pårørende og til slutt inkludere pårørende i rehabiliteringsarbeidet.
Kirkevold (1997) sitt perspektiv vil, kunne benyttes som teoretisk perspektiv og være førende for begrepsfesting av rehabiliteringssykepleierens funksjoner i en bredere rehabiliteringssammenheng. Ytterligere forskning er nødvendig vedrørende hvordan og når de ulike bidrag bør utføres.

-A qualitative study of patients’ experiences of the contribution of nurses in the rehabilitation program
Summary:
Purpose: The primary purpose of the study was to describe patients` perceptions of the nurses` contribution to the rehabilitation process. Secondly the study sought to examine if Kirkevold`s (1997) theoretical perspective on nursing functions in acute rehabilitation of stroke patients is suitable to capture the essence of patients’ experiences in a broader rehabilitation context.
Theoretical background: Central theoretical perspectives and concepts related to patients` needs during rehabilitation, the ideology of rehabilitation, the rehabilitation process, multidisciplinary teamwork and rehabilitation nursing is reviewed. Kirkevold`s (1997) theoretical perspective was also included in the theoretical background.
Method: This was a qualitative hermeneutic study. The method used to collect data was semi structured interviews. Eight participants with experience from a rehabilitation process participated. The data was analysed using four steps of systematic text condensation.
Result: Findings indicate that the nurses were decisive for how the days where experienced and for the result of the patients’ rehabilitation process, and were defined as the foundation of the rehabilitation. At the same time the nurses` contribution was not considered as rehabilitative. The interviews implied an understanding of treatment and training as rehabilitation and ”the other” , i.e. nursing care, as distinct from rehabilitation. The informants wanted more time with the nurses, and wanted the nurses to challenge them in the process. Furthermore, they expressed a wish for negotiating goals in collaboration with patients and relatives.
Conclusion: The nurses are essential for the rehabilitation process, but more competence is needed. Areas for further competence include providing a context conducive for rehabilitation, create social arenas, facilitate communication and multidisciplinary teamwork. Furthermore, challenging patients in the rehabilitation process, negotiate goals in cooperation with patients and relatives and include the relatives in the rehabilitation process were emphasized. Kirkevold`s (1997) perspective may, be used as a theoretical perspective and facilitate conceptualisation of rehabilitation nurses functions in a broader rehabilitation context. More research is needed in order to explicate the process and context of nursing interventions.
