Abstract
Formål:
Masteroppgaven består av to deler; en artikkel i form av en litteraturstudie, og en refleksjonsoppgave. Artikkelen beskriver og kartlegger hva forskningslitteraturen sier om utrygghetsopplevelse for den eldre pasient på sykehus. Refleksjonsoppgaven vil utforske trygghetsbegrepets betydning og posisjon i sykepleiefaglig sammenheng.
Litteraturgjennomgang:
I artikkelen gjøres søk på databaser MedOvid /Cihnal, PsycINFO Swemed+ Norart, ISI Web of Science, Pubmed, Proquest, Geriatric Web og BIBSYS. Refleksjonsoppgaven vurderer tar utgangspunkt fra sentrale sykepleieteoretikere.
Metode:
Litteraturstudien har form av en undersøkelse der inkluderte artikler blant annet blir beskrevet på grunnlag av informanter, metoder, vurdering av funn, resultat, intervensjoner, konklusjon og begrensninger. Sentrale funn blir kategorisert ut fra relevante tema. Refleksjonsoppgaven utforsker og vurderer valgte sykepleieteorier.
Funn:
I artikkelen fremkom som gjennomgående konklusjon at pasientens utrygghetsopplevelse føltes som en indre angst/uro som kunne få fysiologiske og psykologiske konsekvenser. Sykepleieren kunne med tiltak som, god kommunikasjon mellom sykepleier og pasient, tilretteleggelse av ADL - funksjoner, tilpasning av farger og lys, og skjerming mot støy, bidra til økt trygghet. Betydningen av at sykepleieren er tillitskapende i omsorg og viste respekt for den eldres livserfaring og integritet ble også vektlagt. Refleksjonsoppgaven avdekker at trygghet og trygghetsbegrepet både har hatt, og har økende betydning og posisjon i sykepleieteorier. Det foreligger også stor grad av enighet om at med trygghetsbegrepet ble fokus for sykepleien flyttet til pasientperspektivet.
Konklusjon:
Behovet for videre undersøkelser om temaet synliggjøres i artikkelen, grunnet beskjedent omfang av studier som tar opp de betydelige negative konsekvenser utrygghet kan ha for den eldre pasient ved et sykehusopphold. Refleksjonsoppgaven viser til at det oppfordres til videre forskning på for eksempel hvordan sykepleier og pasient betrakter trygghet, og hvordan trygghetsbegrepet blir fremstilt.

Purpose:
This document consists of two parts: an article in form of a literature-review, and a broader theoretical paper. The article gives a description and survey of published studies on experienced (subjective) discomfort for aged patients in hospitals. The theoretical paper aims at exploring the concept of comfort, as well as establishing its position within nursing science.
Presentation of literature:
In the article search was performed at following databases; MedOvid /Cihnal, PsycINFO Swemed+ Norart, ISI Web of Science, Pubmed, Proquest, Geriatric Web og BIBSYS. The theoretical paper is dealing with prominent nursing science theorists.
Method:
The literature review brings a survey in which included studies is described in terms of informants, methods, findings, results, interventions, conclusions and limitations. Major findings are presented according to relevant aspects of the comfort issue. The theoretical paper explores and considers selected nursing theories.
Findings:
In the literature review is seen that the authors mainly describe patient’s feeling of discomfort as an inner anxiety/unease which might have physiological as well as psychological consequences. By means of interventions such as, good nurse/patient communication, well-planned ADL - functions, control of light and colours, and relief of noise, the nurse might contribute to improved conditions of comfort. The beneficial impact of the nurse promoting an atmosphere of confidence when delivering care was underlined.
Also respect of life-experience and integrity of the aged patient was considered as essential. The theoretical paper reveals that the comfort concept has a rising interest within works of theoretical nursing. Further there seems to be a mutual understanding between theorists that a focus shift from the nurse- to the patient’s perspective is the result of renewed interest in comfort.
Conclusion:
The need for further research in the issue of comfort is evident according to the literature review, due to a modest amount og works dealing with the severe consequences resulting from discomfort in aged patients in hospitals. The theoretical paper highlights the need for new emphasis on clarification of the concept of comfort as well as the way it is conceived by nurses.
