Abstract
Studiens hensikt er: Å klargjøre hva autonomi og selv-kontroll innebærer i kontekst av jordmors yrkesetikk, i forholdet mellom jordmor og den fødende i fødselsprosessen. Å få mer kunnskaper om hvordan fødselsens natur innvirker på kvinnens evne til å utøve selv-kontroll i fødelsprosessen. Jeg vil også kartlegge hvilken betydning autonomi og selv-kontroll har for kvinnen i fødselsprosessen i dag.
Teoretisk bakgrunn: Litteraturgjennomgang i relasjon til autonomibegrepet er basert på Haworths presentasjon av autonomi som et deskriptivt begrep og Tranøys beskrivelse av autonomi som et normativt begrep. Reva Rubins kliniske sykepleieteorier utgjør den sykepleiefaglige rammen for min studie.
Metode og design: Oppgaven er et beskrivende og kartleggende litteraturstudie på sykepleiens teoretiske område. Forskningsspørsmålene og funn besvares via relevante litteraturkilder, og blir diskutert i lys av empiri, kliniske studier og egne erfaringer.
Funn: Autonomiprinsippet tillegges betydelig vekt i jordmors verdigrunnlag og lovverk. Jordmor må bruke sin kunnskap om fødselens natur og omgivelsenes påvirkning som grunnlag for ivare-takelsen av kvinnes autonomi og selv-kontroll i fødselen. Fødselens historiske aspekt, kvinnens behov for selv-kontroll, partnerens rolle og barnets rettssikkerhet bør tydeliggjøres i jordmors etiske grunnlag.Klinisk forskning viser at informasjon om prosedyrer, bruk av fødselsteknologi og det informerte samtykke ikke er godt nok ivaretatt i fødselsomsorgen, og bør forbedres. Kliniske studier maner til en mer sensitiv og kritisk holdning til invasive inngrep og bruk av moderne fødselsteknologi. Selv-kontroll er en dimensjon ved autonomi. Forskning viser at jordmors respekt for kvinnen er grunnleggende for kvinnens utøvelse av autonomi og selv-kontroll i fødselen. Samsvar mellom kvinnens behov for hjelp og jordmors omsorgshandlinger er grunnlaget for kvinnens autonomi og selv-kontroll i fødselen. Kliniske studier viser at en mer fleksibel, mindre rutinepreget og personlig omsorg kan øke samsvaret mellom kvinnens forventninger til fødselen og fødselens realiteter. Den flergangsfødendes "fødsels- historie" og behov for sykepleie bør vies større oppmerksomhet i sykepleien til kvinnen.
The intention of the study is :To clarify the meaning of autonomy and self-control in context of ethics in midwifery, in the relation between the midwife and the laboring woman during the birthprocess. To gain more knowledge concerning "the nature of birth" and how "internal forces" influences the woman`s capacity to exercise self-control in the birthprocess. To chart out the importance of autonomy and self-control in women during their birth process.
Theoretical Background : Litterature review in relation to the concept of autonomy is based on Haworths introduction of autonomy as a descriptive term and Tranøys presentation of autonomy as a normative concept. Reva Rubins clinical nursing theories are used as a conceptual framework in this study.
Method: A descriptive theoretical study in the field of nursing. Research questions and findings are answered and discussed in context of my litterature review, empirical theories, clinical studies and my own experiences as a midwife.
Main findings : Autonomy is a fundamental ethical principle in the ethical guidelines for midwifes and in the law. The historical aspect of birth, the woman`s need to exercise control, the role of the labour partner and the human rights of the fetus must be emphasized in the ethical guidelines for midwifes. The obstetric nurse has a professional and moral obligation to use her knowledge concerning "the nature of birth" and important environmental influences, to promote the experience of autonomy and self-control in women during their birth process. Clinical research shows that information about procedures, birth technology and the informed consent is not taken care of good enough, and must be improved in midwifery care. Clinical studies recommends a more sensitive and critical attitude to invasive examinations and the use of modern technololy in delivery. Research results demonstrates that the midwifes respect is a descisive factor, that contributes to autonomy and self-control`in the laboring women. Self-control is a dimension of autonomy. Agreement between the patients caring needs and the midwifes caring activities is a precondition for autonomy and self-control in birth. Clinical studies demonstrates that a more flexible, non-rutine and personalized care can enhance congruence between the womans birth expectations and the realities of birth. The multiparous "birth history" and caring needs must be given a more serious attention by the nurse.
