Abstract
Bakgrunn: En undertype av hydrocephalus er kalt godartet ekstern hydrocephalus (benign external hydrocephalus, BEH). Behandling av BEH er nevrokirurgisk shunt-operasjon eller avventende, konservativ behandling (ingen behandling). Hvorvidt operasjon skal gjennomføres eller ikke er omdiskutert og ikke evidensbasert, men heller basert på preferansen ved den aktuelle avdelingen. Ingen studier har så vidt vi vet sammenlignet effekten av kirurgisk og konservativ behandling. Det er derfor behov for studier som sammenligner opererte og ikke-opererte barn med BEH. Vår hovedoppgave er en slik studie, dvs. oppfølgingsstudie med hovedfokus på nevropsykologisk funksjon. Denne studien ble gjennomført for å (1) undersøke nevropsykologisk og psykososial funksjon hos barn/ungdom diagnostisert med BEH i spedbarnsalder, (2) sammenligne funksjon til opererte og ikke-opererte med BEH, og (3) undersøke hvilke faktorer som predikerer god/dårlig funksjon. Metode: Utvalget består av 86 deltakere i alderen 8-18 år med påvist BEH før fylte 1 år. De ble undersøkt med nevropsykologiske tester, spørreskjemaer om livskvalitet (Pediatric Quality of Life Inventory, PedsQL) og eksekutiv funksjon i dagliglivet (Behavior Rating Inventory of Executive Function, BRIEF). Omtrent 30 % av deltakerne har gjennomgått shunt-operasjon. Operasjonen ble utført som regel før 12. måned. Resultater: (1) BEH-gruppen skårer innenfor det normale variasjonsområdet på alle nevropsykologiske tester og begge spørreskjemaene. Gruppen presterer samtidig lavere enn normgjennomsnittet på flertallet av de nevropsykologiske funksjonsområdene. Barna skårer seg selv generelt høyere enn gjennomsnittet på spørreskjema av livskvalitet. Foreldrenes rapportering, derimot, indikerer at barna har livskvalitet som barn flest. Resultater på BRIEF er generelt ikke forskjellig fra normgrunnlaget. (2) Opererte barn skårer lavere enn ikke-opererte på to tester av psykomotorisk tempo, og på en test av oppmerksomhet og konsentrasjon. Opererte barn, men ikke deres foreldre, rapporterer også lavere skåre på subskalaen Skole på PedsQL enn ikke-opererte barn. Videre skårer en større andel opererte enn ikke- opererte klinisk signifikant svakt (1,5 standard avvik under gjennomsnittet) på tester av eksekutiv funksjon og finmotorikk, og på delskalaen monitoreringsevne (BRIEF). (3) Kjønn, alder, ventrikkelstørrelse og avvik i hodeomkrets predikerer skåre på enkelte tester og på subskalaen Skole på PedsQL. Disse variablene har imidlertid ikke større prediksjonsverdi enn operasjonsstatus. Konklusjon: Barn/unge med BEH i denne studien har subtile kognitive vansker. Et viktig funn er at opererte barn ikke fungerer nevropsykologisk og psykososialt bedre enn ikke-opererte. Ut fra et nevropsykologisk og psykososialt perspektiv gir resultatene ikke støtte for å operere barn med denne tilstanden.
Background: A subtype of hydrocephalus is benign external hydrocephalus (BEH). Treatment of BEH is neurosurgical shunt operation. The alternative to operation is conservative treatment (no treatment). Whether operation should be performed or not is debated and not evidence based, but depends instead on the preference of the current hospital department. As far as we know, no studies have compared operated and not-operated BEH patients. Our master thesis, however, consists of such a study; a follow-up study whose main focus is neuropsychological function. The purpose of the study was to (1) investigate the neuropsychological and psychosocial function in children/adolescents diagnosed with BEH in infancy, (2) compare the function of operated and not-operated children, and (3) examine what factors predict function. Method: The sample consists of 86 participants in the age between 8 and 18 diagnosed with BEH before the age of 1. The participants were examined with neuropsychological tests, a questionnaire of life quality (Pediatric Quality of Life Inventory, PedsQL) and a questionnaire of executive function in daily life (Behavior Rating Inventory of Executive Function, BRIEF). Approximately 30 % of the participants were operated. Operation was generally performed before the age of twelve months. Results: (1) The BEH group performs within the normal range on all neuropsychological tests and both questionnaires. However, the group performs significantly poorer than the norm average on the majority of neuropsychological functions. The children score themselves higher than the norm average on the questionnaire of life quality. The parents report, on the other hand, indicates that the life quality of BEH children is similar to other children without BEH. The results on BRIEF are generally not different from that of the norm average. (2) Operated children perform poorer relative to not-operated children on two tests of psychomotor speed, and on a test of attention and concentration. The operated children, but not their parents, report lower scores than the not-operated children on the subscale School on PedsQL. Furthermore, a larger proportion of operated children compared to not-operated children score clinically significantly weak (1,5 standard deviation below the mean) on tests of executive function, fine motor function, and on the subscale Monitoring (BRIEF). (3) Gender, age, ventricle size and deviation of head circumference predict score on certain tests and on the subscale School on PedsQL. However, these variables do not have greater prediction value than operation/no operation. Conclusion: Children and adolescents with BEH in this study have subtle cognitive difficulties. An important finding is the fact that operated children neither have better neuropsychological nor psychosocial function compared to the not-operated children. From a neuropsychological and psychosocial perspective, these results do not provide support to operate children with this condition.
